Dear Parents of Sixth Grade Students:
The Ohio Department of Health (ODH) has immunization requirements for students entering 7th grade. Prior to the beginning of next school year your student is required to have confirmation of a Meningococcal and Tdap (tetanus, diphtheria, and pertussis and diphtheria) vaccine on file at school.
Most students will receive these vaccines at their well child check-up. During this visit please have your physician complete the CONFIRMATION OF Meningococcal and Tdap VACCINATION form which can be located at: http://www.mentorschools.net/Downloads/CONFIRMATION%20OF%20Tdap%20and%20Meningococcal%20VACCINATION3.pdf
If you have online access to your students’ medical records you may print out a full copy of the student’s current vaccination record instead of using the confirmation form.
Please submit vaccine confirmation to Mrs. Weber in the clinic prior to the end of this school year. If your student does not have an appointment scheduled until summer please have your student bring confirmation of vaccination on the first day of school.
